Umbilical cord morphology and placental weight in non-aneuploid genetic syndromes

Abstract

OBJECTIVE: To describe gross morphologic features of umbilical cords and placenta! weight from delivered infants with non-aneuploid genetic syndromes. STUDY DESIGN: Live newborns with non-aneuploid genetic s\ndromes delivered at St. Peter's Medical ("'.enter between 1993-1995 were identified. Pathologv, maternal, neonatal, and genetic records were reviewed. Placenta! weight, number of cord vessels, type of cord iwisl (left, right, mixed, absent), type of cord insertion (disc, marginal, velamentons}. gestational age at delivery, and genetic diagnosis were recorded. The findings were compared with a database of 5,864 placentas from all singleton gestations delivered al St. Peter's Medical Center in UI93. Statistical analvsis was by t-test. Significance was set at p<0.05. RESULTS: Nineteen newborns with non-aneuploid genetic suidromes were identified. Five had Beckwith-Wiedemann, five had skeletal dysplasias. and the rest had one of the following: Cardionnopathy-MicrocephaK svndrome, chromosome deletion syndrome, cystic fibrosis, Epidermohsk Bnllosa. (olden bar. Ivemark svndrome, craniosynostosis syndrome, Treacher-Collins syndrome, and Zellweger s\ndrome. The mean gestational age was 35 ±5.5 weeks, and the mean birth weight was 2909 ±1456 grams. The mean placental weight for suidromic newborns was 561 ±178 grams, while for controls, it was 490± 117 grams. The placental weights for the fi\e term Beckwith-Wiedemann infants were signifkanth larger than the control placentas (644 ±118 vs. 490±117 grams; p = .0()3). One curd (5%) had a mixefl twist pattern. Genetic caws (n= 19) Controls (n=iXM) Normal .-ord IS (\W7(} 5111 (87'r ) inserfon 2 vessel cord 1 (5.3%) 27 (0.5rv) Cord twist ratio (L:R) 3.5 :1 7:1 CONCLUSION: For this heterogeneous sample of syndromu nonaneuploid fetuses, none of ihe placenta or umbilical cord morphologic features examined (placental weight. l>pe of core! insertion, number of two \'essel cords, pattern <jf cord twist) was significantly different from the control group. This suggests that abnormal placental-cord morphologv does not contribute to the increased morbiditv seen in pregnancies complicated bv svndr'Hiiic fetuses. The large Beckwith-Wiedemann placentas arc consistent will the I'etal overgrowth seen with this particular svndrome.
