Abstract

OBJECTIVE: To desciibe llu- morphologic placental palhologs findings in plat entas tioin twin gc.slalions tumpliiatcd by preeclampsia. STUDY DESIGN: All twin gestations delr,ercd at St. Peter's Medkal (.einer hum Jannan 1994 to Febrnarv 1995 were idcniilicd. Placentas lioin these pregnancies uith piceclampsia were lompared to noiinolensive twin gestations. Uala i olle( led from the twin pu-edamptic picgnam ies (IT) included hirlh ueighl (BW). chorionicity, geslalional age al deliver, ntlmbcr ol roul esscls. plaienlal hhiosis. Comparisons were helxveen each twin in a twin pail loi- each of the nonpieec lamptic iwin (NPI'l gestations and uerc uimpared to twin PP. Distordanc\ wasdelinedasan innapair dillei dice in hhllnveighl ol'at least < . Anahsis Results reported in mean |SD| odds tatios (OR) with 95% C.I. RESULTS: A total of 152 lu in gi-stalions uere anahycd. ,il uhom loin leen were diagnosed with preeclampsia. The mean gestalional age at deliver for NPP was 35.0(±4.5 weeks versus 35.2 (± 1.5) weeks lor PP with p > 0.05. The prevalence ot prceelampsia in dichorionu twin gestations was 9.8%. (13/132) versus.5.0% (1/20) in mono, hoiioihc luin gcslaliuns: OR 2.1 (95% CI 0.3-17.0). All placentas had 3 vessel, cords. Discordance was observed in 3 dichorionic twin PP dam c was sened in 3 dichoiionn twin PP all ol whom had a lel't cord twist and noiinal < ord insertion. Patholog\ and findings from PP: Characteristics Twin A Twin B Birth weight (grams) 2345.3 (±391.3) 2326 (±436.4) Placenlal weigtht (grams) 380.1 (±70.3) 386.2 (±87.3) Cord insert (velamentuns/marginal vs. normal) 43% (6/14 29% (4/14) Membran, insert (circumarginale vs. normal) 43% (6/14) 21% (3/14) Cord twist (left vs. right) 93% (13/14) 71% (4/14) All comparisons for räch tw.n pair in I'P were nol significant (p > 0.05). The degree and prf stnet ot Hot is maun it and suicvtiai knots were similar in all Iwin PP. as wrrc the presence ot yilhms librosis and privillous fibrin. HOHCMT. the degree of villeins Hbrosis was dillc-rent in 1 of The I 1 luin PP and the degree perivillons fibrin deposition ua.s different in 3 of the 14 twin PP. CONCLUSION: Histologie lésions in twin PP uvre concordanl. Likewise, ihe gross plao:Milal and cord mnrpholocA- ueiv similar in twin PP.
